Parental consent for school to administer medicine

If you require school to administer prescribed medication to your child, then please complete the 'Medicine Parental Consent' form below and return to the school office.

Prescribed medication will only be administered by certified members of staff and in line with our 'Managing Medicines Policy'

Note: Medicines must be in the original container as dispensed by the pharmacy.
